for its users. Our Privacy Policy (“Privacy Policy”) is designed to help you understand how
we collect, use and safeguard the information you provide to us and to assist you in making
informed decisions when using our Service.

For purposes of this Agreement, “Site” refers to the Company’s website, which can be

including our electronic medical record services, practice management services, other operations
workflow solutions and other services provided by us to you.

The terms “we,” “us,” and “our” refer to the Company.“You” refers to you, as a user of our Site
or our Service. By accessing our Site or our Service, you accept our Privacy Policy and Terms of

use and disclosure of your Personal Information as described in this Privacy Policy.

I. INFORMATION WE COLLECT
We collect “Non-Personal Information” and “Personal Information.” Non-Personal Information
includes information that cannot be used to personally identify you, such as anonymous usage
data, general demographic information we may collect, referring/exit pages and URLs, platform
types, preferences you submit and preferences that are generated based on the data you submit
and number of clicks. which you submit to us through the registration process at the Site.
Protected Health Information (and Your Personal Information as necessary) between
You and covered entities (and their business associates) for any permitted purpose,
including, to the extent applicable, care coordination, performance or quality
measurement programs, and risk adjustment, and other treatment, payment or health
care operations purposes.

Bluebook EHR collects certain information from its users in three ways: (a) from Bluebook EHR
Web server logs, (b) with cookies and (c) directly from the user.
(a) IP Addresses (Server Log Information)
An IP address is a number automatically assigned to your computer whenever you access the
internet. All computer identification on the Internet is conducted with IP addresses, which allow
computers and servers to recognize and communicate with each other. Bluebook EHR collects
IP addresses in order to conduct system administration, report aggregate information to
affiliates, subsidiaries, sponsors, advertisers or partners, and to conduct site analysis. Bluebook
EHR will also use IP addresses to identify any users who refuse to comply with Bluebook EHR’s
Terms of Use Agreement, and to identify users who threaten Bluebook EHR’s service, site, user,
clients or others.

(b) Cookies
Bluebook EHR places a text file called a “cookie” in the browser files of your computer. Cookies
are pieces of information that a site transfers to an individual’s hard disk for record keeping
purposes. Bluebook EHR uses cookies during your online session to deliver content specific to
your interests. Cookies allow Bluebook EHR to avoid showing you the same ad or other
message repeatedly. Bluebook EHR Site cookies do not contain personal information. They do
enable Bluebook EHR to relate your use of the site to information that you have specifically and
knowingly provided to Bluebook EHR.
If you do not want cookies to be set on your device, you can adjust the setting of your Internet
browser to reject the setting of all or some cookies and to alert you when a cookie is placed on

(c) Web Analytics
Bluebook EHR may use Google Analytics to understand Bluebook EHR’s site usage. Site usage
information is used to help design, develop, and support Bluebook EHR. To the extent Bluebook
EHR uses Google Analytics, Google receives and stores Bluebook EHR’s contributed site usage
information (such as pages accessed), but it does not receive any individually identifiable or
sensitive information as a part of this process. If you do not want data collected by Google
Analytics, you can use the Google Analytics Opt-out Browser Add-on available on Google’s
website.

(d) Registration (User-Supplied Information)
Bluebook EHR site registration systems require users to give Bluebook EHR contact
information such as their name and e-mail address, and demographic information such
as organization and/or role. The user’s contact information is used to contact the user
when necessary. Users may choose (opt-in) to receive future mailings; see the
choice/opt-in section below. Bluebook EHR uses this data to tailor the user’s experience
at Bluebook EHR, showing the user content that reflects their stated preferences. This
information may be shared with affiliates, subsidiaries, sponsors, advertisers or
partners on an aggregate basis, but personally identifiable information will not be
disclosed without the user’s prior consent

II. CHILDREN’S POLICY

The Site and the Service are not directed to anyone under the age of 13. The Site does not
knowingly collect or solicit information from anyone under the age of 13, or allow anyone
under the age of 13 to sign up for the Service. In the event that we learn that we have
gathered personal information from anyone under the age of 13 without the consent of a
parent or guardian, we will delete that information as soon as possible. If you believe we

Personal Information:

consent. We do share Personal Information with vendors who are performing services for
the Company, such as the servers for our email communications who are provided access
to the user’s email address for purposes of sending emails from us. Those vendors use
your Personal Information only at our direction and in accordance with our Privacy Policy.
In general, the Personal Information you provide to us is used to help us communicate with
you. For example, we use Personal Information to contact users in response to questions,
solicit feedback from users, provide technical support, and inform users about promotional

such Non-Personal Information to our partners, advertisers and other third parties at our

Bluebook EHR gives users the opportunity to opt-in to receive communications from
Bluebook EHR at the point where Bluebook EHR requests information about the user.

Bluebook EHR’s database in order to stop receiving communications or Bluebook EHR’s
service; for reviewing, changing and modifying information previously provided (subject
to reasonable frequency limitations, which Bluebook EHR has established as three or

As mentioned in the Terms of Use Agreement, ‘Modification of These Terms of Use’
section, Bluebook EHR reserves the right to change the terms, conditions, and notices
under which the Bluebook EHR Site is offered, including but not limited to any changes
associated with the use of the Bluebook EHR Site now or in the future. You are
responsible for regularly reviewing these terms and conditions. However, please note
that we will give Web users the opportunity to opt-out or prohibit new/unrelated uses of
their personal information.

VI. LINKS TO OTHER WEBSITES

As part of the Service, we may provide links to or compatibility with other websites or
